Welke veranderingen zijn ziektepatronen ondergaan?

In de laatste decennia is er veel veranderd in de hedendaagse ziektepatronen. Dit komt onder andere door de vooruitgang in de ontwikkeling van medicijnen. Vandaag de dag krijgen meer mensen te maken met hartfalen en vaatziektes, dan met infecties en ondervoeding. Samenhangend met de wijziging in ziektepatronen, wordt er ook anders tegen ziektes aangekeken. Wetenschappers ontdekten dat er een sterk verband tussen onze emoties en de biologische processen in ons lichaam bestaat. Onze gedachten, gevoelens en persoonlijkheden beïnvloeden deze processen, wat ervoor zorgt dat twee mensen die dezelfde ziekte krijgen beide een verschillend ziekteverloop doormaken. Ook stress is van invloed op onze gezondheid en kan er zelfs voor zorgen dat we ons ziek voelen. Stress kan echter leiden tot aanpassing van het lichaam in bepaalde situaties om zo te kunnen overleven.

Er zijn ook verschillen op te merken tussen mens en dier, voornamelijk op de manier waarop zij stress ervaren. Dieren ervaren stress bij acute fysieke crisissituaties. Het lichaam blijkt prima in staat te zijn om met dit soort stress om te gaan. Mensen daarentegen ervaren vaak chronische fysieke stress. Het lichaam kan redelijk goed omgaan met deze vorm van stress.

Naast acute en chronische fysieke stress bestaat er nog een derde vorm: psychische en sociale stress. Deze vorm van stress is kenmerkend voor mensen en bestaat eigenlijk alleen in ons hoofd. Mensen kunnen heftige emoties ervaren die het resultaat zijn van enkel gedachten. Menselijke lichamen kunnen dezelfde reacties geven op psychische en sociale stress als op fysieke stress. Het menselijk lichaam kan zich heel goed aanpassen om te kunnen omgaan met acute stress die een korte tijd aanhoudt. Dit is echter anders wanneer de stress chronisch lijkt te zijn en we ons stresssysteem vaak en lang activeren.

Het lichaam is niet ingesteld op chronische stress, want de fysiologische verdedigingsmechanismen zijn oorspronkelijk bedoeld om het lichaam in staat te stellen snel en efficiënt te reageren op een plotselinge bedreiging. Gedurende een zekere tijd zal het lichaam weerstand bieden aan de psychologische stress, maar daarna volgt onvermijdelijk uitputting. Er kunnen hierdoor stressgerelateerde ziekten ontstaan.

Het lichaam is voortdurend bezig om een staat van homeostase te bereiken. Het lichaam bestaat uit verschillende mechanismen en elke mechanisme heeft een optimale waarde. Homeostase betekent dat het lichaam volledig in balans is, dus dat alle mechanismen hun optimale waarde hebben. Het lichaam is altijd bezig om deze balans te bereiken. Er zijn echter een aantal dingen die het lichaam uit de homeostase halen. Dit noemen we stressfactoren en zijn gebeurtenissen die buiten het lichaam plaatsvinden. Het lichaam reageert hierop door middel van een stressreactie, het gaat activiteiten ondernemen om de homeostase weer te bereiken.

Een stressfactor hoeft echter niet alleen datgene te zijn wat op dit moment plaatsvindt. Mensen kunnen een stressreactie uitlokken door na te denken over potentiële stressfactoren. De stressreactie wordt dus niet alleen opgewekt bij fysieke of psychologische dreigingen, maar ook in afwachting hiervan. Dit werd voor het eerst ontdekt door Selye, ongeveer vijfenzestig jaar geleden. Selye deed een onderzoek bij ratten. Zijn ratten kregen dagelijks een injectie met een extract van een eierstok. Doordat hij wat onhandig was, vielen ze echter vaak en dan moest hij ze de halve ochtend zoeken voordat hij ze kon injecteren. Aan het eind van het onderzoek bleek dat de ratten last hadden van vergrote bijnieren en maagzweren. Om uit te sluiten dat deze verschijnselen het resultaat waren van de injecties gebruikte Selye een controlegroep die geïnjecteerd werden met een zoutoplossing. Ook deze ratten vielen regelmatig op de grond en werden opgejaagd door Selye in een poging hun te pakken te krijgen. De controlegroep bleek dezelfde verschijnselen te vertonen als de experimentele groep, wat Selye tot de conclusie bracht dat deze lichamelijke verschijnselen werden veroorzaakt door de stress die ontstond als hij de gevallen ratten achterna zat.

Selye kwam tot twee conclusies:

  • Het lichaam hanteert gelijke stressreacties voor uiteenlopende stressfactoren. Selye noemde dit het algemene aanpassingssyndroom, nu wordt dit verschijnsel de stressreactie genoemd.
  • Stressfactoren die te lang aanhouden kunnen tot ziekte leiden.

Waarom is de theorie van homeostase uitgebreid met de theorie van allostase?

Het oorspronkelijke concept van homeostase was gebaseerd op twee ideeën:

  • Er is één optimaal niveau voor alle meetbare dingen in het lichaam.
  • Je bereikt dit optimale niveau door een lokaal regulerend mechanisme.

Dit concept bleek echter niet te kloppen en is daarom uitgebreid met de theorie van allostase. Met betrekking tot het eerste idee houdt dit in dat er verschillende optimale niveaus zijn, afhankelijk van wat je aan het doen bent. De optimale waarde voor hartslag is veel lager als je ligt te slapen, dan wanneer je aan het trainen bent voor de marathon. Voor het tweede idee houdt dit in dat er niet één manier is waarop de ideale waarde bereikt kan worden, maar dat dit via verschillende wegen kan. Iedere manier heeft zijn eigen consequenties.

Grof gezegd is het verschil dat homeostase bij een tekort alleen het verantwoordelijke mechanisme aanspreekt en dat allostase ervoor zorgt dat de hersenen veranderingen in het hele lichaam, en daarbij vaak ook in het gedrag van een persoon, teweeg brengt.

Het lichaam hoeft overigens niet alle complexe regulerende processen stop te zetten om een bepaalde waarde weer te herstellen. Bovendien is het lichaam in staat om te anticiperen op waarden die af zullen gaan wijken van hun optimale niveau. De hersenen kunnen vooraf al bepaalde lichaamsfuncties aansturen, die er voor zullen zorgen dat een bepaalde waarde niet zijn optimale niveau zal verliezen. Dit komt overeen met het feit dat het lichaam kan reageren op een stressfactor voordat deze daadwerkelijk plaatsvindt.

Wat is in de theorie van allostase een stressfactor?

In de theorie van allostase wordt een stressfactor gezien als al datgene wat je uit je allostatische balans haalt en de stressreactie als de poging van je lichaam om de allostase te herstellen. Hoewel het gebruikelijk is om te denken dat specifieke uitdagingen voor het lichaam om specifieke aanpassingen vragen, geeft iedere stressfactor dezelfde stressreactie af.

Bij mensen ligt het kernpunt van de stressreactie in het feit dat de spieren in het lichaam veel harder dan normaal moeten werken. Ten eerste moet er glucose worden gemobiliseerd, wat zorgt voor energie. De glucose wordt vervolgens door het opvoeren van de hartslag, bloeddruk en ademhaling razendsnel door het lichaam vervoerd. Het sympatische systeem wordt geactiveerd, terwijl andere processen in het lichaam juist worden stopgezet. Voorbeelden hiervan zijn de spijsvertering, groei en voortplanting.

Ook het immuunsysteem zal tijdelijk worden onderdrukt. De energie die hiermee bespaard wordt, zal worden ingezet om het sympatische systeem harder te laten werken. Het parasympatische systeem wordt tijdelijk uitgezet. Ook kan tijdens acute stress de waarneming van pijn uitgeschakeld worden.

Tijdens acute stress vindt er tevens een verandering plaats in cognitieve en sensorische vaardigheden. Bepaalde delen van het geheugen verbeteren, waardoor je razendsnel kan herinneren of je al eerder in een soortgelijke situatie bent geweest en hoe je eruit kan komen. Bovendien worden je zintuigen scherper.

Selye ontwierp een drie fasen theorie over stressreacties:

  1. De eerste fase is de alarmfase, waarin de stressfactor wordt opgemerkt.
  2. De tweede fase is de fase van aanpassing of weerstand, waarin het systeem voor de stressreactie is gemobiliseerd en het lichaam een poging doet om terug te komen in de allostatische balans.
  3. Bij aanhoudende stress kan de derde fase van uitputting ingaan.

Bij de derde fase ging Selye echter de fout in door te denken dat je ziek werd doordat de hormonen die worden afgescheiden tijdens de stressreacties op raakten. Dit kan echter niet het geval zijn, omdat deze hormonen zo cruciaal zijn dat personen nooit zonder komen te zitten. De stressreactie kan een schadelijk effect hebben op het lichaam als hij voldoende geactiveerd wordt. Het is vooral schadelijk als de stressfactor psychologisch is. Vooral bij langdurige (psychologische) stress kunnen de gevolgen groot zijn. Doordat zo vaak de energie vrij komt als reactie op de stress heeft het lichaam niet de tijd om te herstellen en opnieuw reserves op te bouwen en hierdoor zal het lichaam sneller uitgeput raken. Mensen die onder chronische stress staan hebben ook sneller last van:

  • stoornissen in de voortplanting.
  • Diabetes.
  • Hoge bloeddruk.
  • Infectieziekten.
  • Bepaalde hersenfuncties kunnen worden aangetast.

Wanneer je veel stress hebt en dus vaak stresshormonen uitscheidt, is het iedere keer weer moeilijker om de balans van allostase te vinden. De volgende problemen kunnen ontstaan:

  • Het kost heel veel energie om telkens weer deze balans te proberen te vinden. Deze energie kan niet meer gebruikt worden voor processen die op de lange termijn plaatsvinden.
  • Door het gebruik van stresshormonen wordt de stressfactor wel verholpen, maar hierdoor worden wel vaak kleine processen in het lichaam uit balans gebracht. Zo wordt er nog steeds geen volledige allostase bereikt.
  • Ziekte kan ook ontstaan wanneer de stressreactie te langzaam wordt uitgezet, of wanneer verschillende onderdelen op verschillende tijden worden uitgezet.

Het hebben van een stressreactie is belangrijk, zoals blijkt uit het bestuderen van mensen die de stressreactie vanwege een ziekte niet hebben. Twee van zulke ziektes zijn:

  • de ziekte van Addison.
  • Shy-drager-syndroom.

Bij deze ziektes worden twee belangrijke hormonen voor de stressreactie niet afgescheiden.

Als je herhaaldelijk je stressreactie activeert of wanneer je moeite hebt het weer uit te schakelen als de stressvolle situatie voorbij is, is het mogelijk dat het uiteindelijk schadelijk is voor het lichaam. Het is echter niet de stress of de stressfactor die je ziek maakt. Chronische stressfactoren of stressfactoren die vaak terugkomen vergroten enkel het risico om ziek te worden. Ook vergroten zij de kans dat verdedigingsmechanismen de bestrijding van een ziekte niet goed aankunnen. In dit hoofdstuk wordt de invloed van stress op de groei besproken.

Door te eten groeien mensen. Cellen splitsen zich, worden groter en maken nieuwe proteïnen. Botten worden langer, doordat kraakbeencellen aan het uiteinde van het bot naar de schacht migreren en daar harde botten worden. Vet wordt vervangen door spiermassa, de stem wordt lager, er groeit haar op allerlei plaatsen van het lichaam, borsten ontwikkelen zich en testes worden groter. De energie die glucose maakt, laat al deze processen draaien. Een groeihormoon stuurt deze processen aan. Groeihormonen kunnen op een directe manier op cellen in het lichaam werken. Soms werken groeihormonen op een indirecte manier en zorgt het voor de uitscheiding van de hormonen somatomedinen. Deze doen vervolgens het werk. Schildklierhormonen stimuleren de groei door hormonen uit te scheiden, die botten meer ontvankelijk maken voor somatomedinen.

In this chapter, a number of principles that are important for human functioning will be discussed.

What changes have disease patterns undergone?

In recent decades, much has changed in contemporary disease patterns. This is partly due to the progress in the development of medicines. Today, more people are dealing with heart failure and vascular disease than with infections and malnutrition. Associated with the change in disease patterns, diseases are also viewed differently. Scientists discovered that there is a strong link between our emotions and the biological processes in our body. Our thoughts, feelings and personalities influence these processes, causing two people who get the same disease to go through different disease courses. Stress also affects our health and can even make us feel ill. However, stress can lead to the body adapting in certain situations in order to survive.

There are also differences between humans and animals, especially in the way they experience stress. Animals experience stress in acute physical crisis situations. The body appears to be perfectly capable of dealing with this type of stress. People, on the other hand, often experience chronic physical stress. The body copes fairly well with this type of stress.

In addition to acute and chronic physical stress, there is a third form: psychological and social stress. This form of stress is characteristic of humans and actually only exists in our heads. People can experience strong emotions that are the result of just thoughts. Human bodies can respond in the same way to psychological and social stress as to physical stress. The human body can adapt very well to deal with acute stress that lasts for a short time. However, this is different when the stress appears to be chronic and we activate our stress system often and for a long time.

The body is not geared to chronic stress, because its physiological defense mechanisms are originally designed to enable the body to respond quickly and efficiently to a sudden threat. For some time the body will resist the psychological stress, but then exhaustion will inevitably follow. This can cause stress-related illnesses.

The body is constantly trying to reach a state of homeostasis. The body consists of different mechanisms and each mechanism has an optimal value. Homeostasis means that the body is completely in balance, so that all mechanisms have their optimal value. The body is always trying to achieve this balance. However, there are a number of things that take the body out of homeostasis. We call these stress factors and are events that take place outside the body. The body responds to this by means of a stress response, it will undertake activities to regain homeostasis.

However, a stressor does not have to be just what is happening right now. People can trigger a stress response by thinking about potential stressors. The stress response is therefore not only triggered by physical or psychological threats, but also in anticipation of them. This was first discovered by Selye about sixty-five years ago. Selye did a study in rats. His rats were injected daily with an extract from an ovary. However, being clumsy, they often fell and he had to search for them half the morning before he could inject them. At the end of the study, it was found that the rats had enlarged adrenal glands and stomach ulcers. To rule out that these symptoms were the result of the injections, Selye used a control group injected with saline solution. These rats, too, regularly fell to the ground and were chased by Selye in an attempt to get them. The control group showed the same symptoms as the experimental group, which led Selye to conclude that these physical symptoms were caused by the stress that arose when he chased the fallen rats.

Selye came to two conclusions:

  • The body uses the same stress responses for various stress factors. Selye called this the general adaptation syndrome, now this phenomenon is called the stress response.
  • Stress factors that persist for too long can lead to illness.

Why has the theory of homeostasis been expanded to include the theory of allostasis?

The original concept of homeostasis was based on two ideas:

  • There is one optimal level for all measurable things in the body.
  • You reach this optimal level through a local regulatory mechanism.

However, this concept turned out to be incorrect and has therefore been expanded to include the theory of allostasis. With regard to the first idea, this implies that there are different optimal levels depending on what you are doing. The optimal value for heart rate is much lower when you are sleeping than when you are training for the marathon. For the second idea, this means that there is not one way in which the ideal value can be reached, but that this can be done in different ways. Each way has its own consequences.

Roughly speaking, the difference is that homeostasis in a deficiency only appeals to the responsible mechanism and that allostasis causes the brain to bring about changes throughout the body, and often also in the behavior of a person.

Incidentally, the body does not have to stop all complex regulatory processes in order to restore a certain value. In addition, the body is able to anticipate values ​​that will deviate from their optimal level. The brain can already control certain bodily functions, which will ensure that a certain value will not lose its optimal level. This is in line with the fact that the body can react to a stressor before it actually takes place.

What is a stressor in the theory of allostasis?

In the theory of allostasis, a stress factor is seen as anything that takes you out of your allostatic balance and the stress response as your body's attempt to restore allostasis. Although it is common to think that specific challenges for the body require specific adjustments, each stressor triggers the same stress response.

In humans, the core of the stress response lies in the fact that the muscles in the body have to work much harder than normal. Firstly, glucose must be mobilized, which provides energy. The glucose is then transported through the body at lightning speed by increasing the heart rate, blood pressure and respiration. The sympathetic system is activated, while other processes in the body are stopped. Examples of this are digestion, growth and reproduction.

The immune system will also be temporarily suppressed. The energy that is saved with this will be used to make the sympathetic system work harder. The parasympathetic system is temporarily turned off. Pain perception can also be turned off during acute stress.

During acute stress there is also a change in cognitive and sensory skills. Improve certain parts of the memory, so you can quickly remember whether you have been in a similar situation before and how to get out. In addition, your senses become sharper.

Selye developed a three-phase theory of stress responses:

  1. The first phase is the alarm phase, in which the stress factor is noticed.
  2. The second phase is the adaptation or resistance phase, in which the stress response system is mobilized and the body makes an attempt to return to allostatic balance.
  3. With persistent stress, the third phase of exhaustion can enter.

In the third phase, however, Selye made the mistake of thinking that you got sick because the hormones that are secreted during the stress reactions ran out. However, this cannot be the case because these hormones are so crucial that individuals will never run out. The stress response can have a harmful effect on the body if it is sufficiently activated. The stress response is especially harmful if the stress factor is psychological. Especially with long-term (psychological) stress and when you often suffer from stress, the consequences can be major. Because you release your energy so often in response to the stress, the body does not have time to recover and rebuild reserves and the body will become exhausted more quickly. People who are under chronic stress are also more likely to suffer from:

  • reproductive disorders.
  • Diabetes.
  • High bloodpressure.
  • Infectious Diseases.
  • Certain brain functions can be affected.

When you are under a lot of stress and therefore often secrete stress hormones, it is every time more difficult to find the balance of allostasis. The following problems can arise:

  • It takes a lot of energy to keep trying to find this balance. This energy can no longer be used for processes that take place in the long term.
  • The stress factor is remedied by the use of stress hormones, but this often unbalances small processes in the body. Thus, complete allostasis is still not achieved.
  • Illness can also develop when the stress response is turned off too slowly, or when different parts are turned off at different times.

Having a stress response is important, as shown by studying people who don't have the stress response because of an illness. Two such diseases are:

  • Addison's disease.
  • Shy carrier syndrome.

In these diseases, two important hormones for the stress response are not secreted.

If you repeatedly activate your stress response or have difficulty turning the stress response off when the stressful situation is over, it is possible that the stress response is ultimately harmful to the body. However, it is not the stress or the stressor that makes you sick. Chronic stress factors or stress factors that come back often only increase the risk of becoming ill. They also increase the likelihood that defense mechanisms cannot cope with a disease properly. There are several steps between getting a disease and actually getting sick. This also creates individual differences.

  • Competentietools: voor meer werkplezier en energie en voor betere prestaties tijdens studie of werk kan je gebruik maken van de pagina's voor vaardigheden en competenties.
  • Samenvattingen: de studiehulp voor Rechten & Juridische opleidingen is sinds de zomer van 2023 volledig te vinden op JoHo WorldSupporter.org. Voor de studies Pedagogiek en Psychologie kan je ook in 2024 nog op JoHo.org terecht.
  • Projecten: sinds het begin van 2023 is Bless the Children, samen met JoHo, weer begonnen om de slum tours nieuw leven in te blazen na de langdurige coronastop. Inmiddels draaien de sloppentours weer volop en worden er weer nieuwe tourmoeders uit deze sloppen opgeleid om de tours te gaan leiden. In het najaar van 2023 is ook een aantal grote dozen met JoHo reiskringloop materialen naar de Filipijnen verscheept. Bless the Children heeft daarmee in het net geopende kantoortje in Baseco, waar de sloppentour eindigt, een weggeef- en kringloopwinkel geopend.

Vacatures, Verzekeringe en vertrek naar buitenland:

World of JoHo:

  • Leiden: de verbouwing van het Leidse JoHo pand loopt lichte vertraging op, maar nadert het einde. Naar verwachting zullen eind februari de deuren weer geopend kunnen worden.
  • Den Haag: aangezien het monumentale JoHo pand in Den Haag door de gemeente noodgedwongen wordt afgebroken en herbouwd, zal JoHo gedurende die periode gehuisvest zijn in de Leidse vestiging.
  • Medewerkers: met name op het gebied van studiehulpcoördinatie, internationale samenwerking en internationale verzekeringen wordt nog gezocht naar versterking!
